Shoe Assistance Request Form

Every student deserves to feel confident and prepared for the path ahead.

The Avyn Craig Memorial Foundation accepts shoe-assistance requests from parents, guardians, educators, counselors, school staff, and approved community partners.

Requests are reviewed with care, respect, and confidentiality. Submitting a request does not guarantee assistance. Support is based on eligibility, available funding, inventory, location, and current program capacity.
